Starting the application process for Social Security Disability (SSD) benefits can seem like a daunting task as there are several forms that need to be filed as part of the application.  This post is a high-level overview of some of the Social Security Disability Forms that you will need when you file an application for benefits.

The necessary SSD forms

There are several forms that need to be filled out by applicants depending on whether they are applying for Social Security Disability (SSD) or Supplemental Security Income (SSI) benefits. The forms involved in a typical application for SSD and SSI benefits include:

  • SSA-16-BK – Application for Disability Insurance Benefits (SSDI only)

This form consists of 33 questions. Many of these questions are routine, like your address, names of your past employers, and other information that will let the SSA determine which federal benefits you are entitled to receive.

  • SSA-8000-BK – Application for Supplemental Security Income (SSI only)

This form is quite long and consists of 65 questions. Most of the questions are aimed at finding out your income and assets so that the SSA can determine whether you meet the financial qualifications for SSI.

  • SSA-3368-BK – Adult Disability Report

This form asks you to describe the nature of your disability, whether your impairments are mental or physical, the date when your symptoms began, what type of medical treatment you have received, and the medical facilities at which you received such treatment.

  • SSA-3369-BK – Work History Report

This form asks for information that will help the claims examiner understand how your illnesses, injuries, or conditions affect your ability to do work. It also tells them about the kinds of work you have performed in past, the skills you needed to do that work, and the physical and mental requirements of each of your past positions.

  • SSA-827-BK – Authorization to Disclose Information to the SSA

This form is a blanket authorization that gives the SSA the right to retrieve all relevant educational and medical records that may support or deny the validity of your claim for Social Security disability benefits.
